and Jeremy Bailon.
Services will be at 2:00 pm Saturday, June 14, 2014 at Mountain View
Funeral Home Chapel, 7900 E. Main St., Mesa, AZ. Memorials can be made in
Jerry's honor to the Alzheimer's Association, 1028 E. McDowell Rd,
Phoenix, AZ 85006.
